Intro Form: Students with Special Needs or A Disability

We are delighted to serve any and all families who desire to worship at Quest. We believe that everyone belongs, and we mean that in every way. Serving students with special needs and disabilities well is crucial to our community's health and showing Jesus's love. 

This information is extremely important for Quest Kids staff and volunteers to serve your child and your family in the most helpful and person-centered ways. Please utilize the bottom box if you have any other information to share with staff. 

This can look like: "they love to do (x)", or, "I have a hard time understanding (x), but (y) helps me stay involved!" Include information that is important for Quest staff and volunteers to know while serving your child and family.

Please include 4-8 strengths of your child (i.e: love to play an instrument, plays well with others, care about (x), friendly with (x).

Please include what some challenges are for your child. I.e., sensory overload when (x) happens, and complex directions are hard.
Please also include how your child thinks: is abstract helpful? More black and white? What feelings can be trigger points? What are some physical or emotional triggers that we should be aware of?

Not enough physical activity, unclear expectations, too many directions, too strong or light of directness from a teacher/ buddy? Etc.
